Objectives of the CDC+ Program

CDC+ empowers individuals with disabilities and the elderly to maximize Medicaid resources by providing choice, control, and cost-effective care options. It enables participants to make personalized decisions, access support services, and remain in their homes, fostering independence, satisfaction, and improved quality of care.

What is CDC+?

Consumer-Directed Care Plus (CDC+) empowers individuals in the Aged and Disabled Adult Waiver to choose personalized long-term care services, enhancing their quality of life. Participants manage budgets, hire skilled workers (including family or friends), and receive guidance from consultants to meet daily needs and achieve their goals.

Eligibility for CDC+

To qualify for the CDC+ program, participants must have a medical need for care, be of any age (birth to senior), and receive services under Florida’s Aged and Disabled or Traumatic Brain/Spinal Cord Injury Waivers while meeting Medicaid’s financial eligibility requirements.
