CDC+ Service

Consumer-Directed Care Plus (CDC+) Program is a self-directed waiver/program for people participating in the Aged and Disabled Adult Waiver. CDC+ is a long-term care program that is an alternative to the Medicaid Home and Community-Based Services (HCBS) Medicaid Waiver.

This program allows individuals to improve the quality of their lives by being empowered to choose the support and care services that will meet their long-term care needs and help them achieve their goals. This program allows participants to hire skilled care workers, support providers, and vendors. This may include family members or friends – to help them with daily needs such as cleaning, cooking, and getting dressed.

BACKGROUND SCREENING AND CLEARINGHOUSE

CDC+ consumers are in charge of employing service providers or support staff. The consumers/representatives must register in the “Clearinghouse” to commence a background screening on prospective direct care providers, rescreen for current providers, and obtain the screening results from the online database.


The “Clearinghouse” is a statewide screening database. It is a platform used as a single data source for background screening results for persons screened for employment or licensure who provide direct care services to children, the elderly, and persons with disabilities. All APD customers will be able to use the Clearinghouse.
