Florida Consumer-Directed Care Plus (CDC+)
CDC+ Service
This program allows individuals to improve the quality of their lives by being empowered to choose the support and care services that will meet their long-term care needs and help them achieve their goals. This program allows participants to hire skilled care workers, support providers, and vendors. This may include family members or friends – to help them with daily needs such as cleaning, cooking, and getting dressed.
ELIGIBILITY
The eligibility requirements of the CDC+ Program are:
- The participant must have a medical need for care
- The participant must be 60+ years old.
- The participants must receive services under Florida’s Aged and Disabled or Traumatic Brain / Spinal Cord Injury Waivers and be financially qualified for Medicaid.
To be financially qualified for Medicaid, the applicant must have countable assets limited to $2,000 for a single applicant or $3,000 for a married applicant if both parties are applying. At most, their monthly income should be $2,163 for a single applicant or $4,326 for a married couple applying. OBJECTIVES
The objectives of the CDC+ Program are:
- The goal is to allow consumers of disability and long-term care services to make more efficient use of Medicaid resources by providing significant choice and control to program participants.
- To empower aged persons, people with disabilities, and their families to make beneficial choices about purchases from both formal and informal sources that best meet their needs.
- To provide aged persons, people with disabilities, and their families with the ability to make cost-effective purchases.
CDC+ offers various support services to help people remain living in their homes or group homes. Consumer-directed Care Plus gives consumers greater control and flexibility.
Steps to Enrolling
The eligibility requirements of the CDC+ Program are:
- Enroll in DD/HCBS iBudget Waiver.
- You must live in your own home or family home.
- Select a representative (if needed) and complete a Representative Agreement.
- Take the CDC+ Initial Training.
- Pass the Initial Training Readiness Review with a score of 85% or higher.
- Obtain a copy of the approved cost and support plans from the CDC+ consultant.
- Submit the application, enrollment documents, and direct deposit form to the consultant.
- The State Office sends the Budget Authorization Form (BAF) containing the consumer ID, initial budget, and anticipated start date after processing documents.
- Allow a minimum of three weeks for processing

BACKGROUND SCREENING AND CLEARINGHOUSE
CDC+ consumers are in charge of employing service providers or support staff. The consumers/representatives must register in the “Clearinghouse” to commence a background screening on prospective direct care providers, rescreen for current providers, and obtain the screening results from the online database.
The “Clearinghouse” is a statewide screening database. It is a platform used as a single data source for background screening results for persons screened for employment or licensure who provide direct care services to children, the elderly, and persons with disabilities. All APD customers will be able to use the Clearinghouse.
